Well you seem to have 3 parties. The woman, the non-humanoid thing which I assume is a prop or computer generated, and the other 3 guys.

The woman does not seem to be enjoying herself, and the 3 guys do (possibly in a sadistic manner). I can't help but think this then might be some sort of rape roleplay, the milk possibly symbolizing a facial, or perhaps just some random form of humiliation / torture.

The non-human thing.. thar's gotta be a computer addition or a well designed prop. It might represent a male being forced to have sex with the woman (in a few minutes after this photo was taken) seeing as no man would actually object to it, they had to resort to a prop.

Or perhaps in a completley non sexual manner, this could have just been some good old fashion fun taken out of context. The look on her face may have just come from shock as milk is poured on her, while the man on the left was just taking part in this mild prank. That leaves the man in the middle and chewbaca left, for which I have no idea.

Some level of photoshopping is present, but indeed this picture does raise one's eyebrows.
