First class on short-range flight hardly deserves the name - your seat may have an extra 2-inches, no big deal.
On an intercontinental flight you'll at the least have a chair that can be turned into a normal bed and probably curtains as well. Some can be re-arranged so you face other people and construct an actual table - and since few people travel first class, you can easily conduct a business meeting even. It's not really intended for the average traveler and of course makes no sense on short trips with the availability of private jets.
It's also often used by government officials (Presidents, foreign ministers, etc) of countries who don't operate long-distance planes for them.
