I get some keyboard+mouse tendinitis, so did some research on gamepads. The WoW mod mentioned absolutely did not work for me. It creates a rediculous number of macros on your hotbars, and it is impossible to switch smoothly from gamepad to keyboard+mouse for intense fights. I found the best solution was:

Add gamepad / joystick support to any game - Pinnacle Game Profiler

I am a priest, I mapped the D-Pad to target F2-F5 in group. I mapped the Left Shoulder Button to Shift, and the Right Shoulder button to Ctrl. I mapped the A-Y buttons to numbers 1-4. Thus I ended up with eight buttons (1-4 and F2-F5), with two possible modifiers (shift and ctrl). Thus, about 24 buttons. The left trigger is left click on the mouse, and the right trigger is right click (duh). The left joystick depress is target nearest, and the right joystick depress is jump. I could do every fight in MC and BWL, excluding razorgore with the gamepad. If things got intense, I could easily drop it and switch to mouse+keyboard as needed.

It made a significant difference in my tendinitis. I've used it a ton in BC Beta, it works great for soloing, and most 5-man groups. I'm a priest, and have no problems healing, but my dps contribution is kinda poor when I'm using the gamepad (usually limited to dots and wand). I would also note that Ventrilo or Teamspeak are also very important.

Although I have a hard time using a joypad in WOW (it just doesn't feel right), I can see how that Pinnacle program would work wonders in a lot of PC games which are designed for gamepads but have crappy in game configuration for controllers, seems like a lot of console ports are like that.

Not really something you can probably do while on a treadmill though, unfortunately.

Playing WoW with a 360 Controller:
L Trigger = ALT
R Trigger = CTRL
L Button = Rotate Camera 90 degrees
R Button = Rotate Camera -90 degrees
ABYX = Jump, Strafe right, I forgot, Strafe left
CTRL-ABYX = Buttons 1-4
ALT-ABYX = Buttons 5-8
SHIFT-ABYX = Target person 1 in your party, person 2, person 3, person 4
D-Pad = Buttons 9-12
Ctrl-D-Pad = Buttons 13 - 16
Alt-D-Pad =Buttons 17 - 20
Shift-D-Pad = Buttons 21-24
Analog Stick 1 = Movement, pressing it down targets self.
Start button = Shift

Works fine for 5 Mans and other PvE shit, sucks ass of course, in pvp.

Interesting. Do you by any chance have any perspective on a comparison between using a gamepad for tendinitis or a graphics tablet in a game like WoW? I recently bought a tablet, but while it I totally love it for windows mouse operation, it keeps being an awkward tool for games/wow, even when I'm doing mundane things like grinding faction. It works pretty well for the tendinitis though and I'm still sticking with it for non-serious gameplay, but if a gamepad can provide the same relief with better game control, I might try it out.

I don't even leave my chair to shit anymore. Currently I'm covered up to my chest in nearly a years worth of refuse. I'm getting worried though as lately my feeding tube has been bringing me regular eggo waffles instead of the butter-flavored ones.

I agree. A lot of things are hella boring in life, what are people going to do carry a laptop with WOW/EQ2 etc for ANYTHING they do? Dang, control the MMO addiction or quit or the following could be you:
