Modern video cards have a molex(same power connection as CD or hard drives) connector on the card. Make sure you have this plugged in.

I'll take a pass on the second part of your question, I don't really know as I have never seen this problem.

Mine came loose once (radeon 9800pro) and it put a big ass "PLUG IN THE POWER TO YOUR CARD" msg on the monitor o.O.
