When I was a kid one of my uncles bought two of those creepy looking mask faces for my brother and I, never seen them anywhere else before that video. There are three plastic holes on the back for you to put your thumb, index, and middlefinger in to contort the rubber face in crazy ways. Pretty creepy when attached to that machine.
