I thought Rallos Zek was required for Sol Ro - you have access to Sol Ro's Tower granted for killing Donkey Zek Jr.'s
Also, the gap in your PoE --> PoT chart (what Furor mentioned above):
Vegarlson (Plane of Earth) --> unannounced NPC/Quest --> Ragrax, Stronghold of Twelve
It seems right now that only the Plane of Earth has more than one zone which just doesn't seem right. Either the devs have hid valuable information from the parsers (B planes that no one has found) or only one of the elemental planes actually has two parts to it.
