Soulfire, without a doubt. I would say Fiery Avenger, except that I never finished the goddamn thing. Vox was soooo stingy with that book that I just gave up eventually. But for a while there back during the first year of release, Soulfire + fbss was just awesome.

As a few have said already, pre-nerf Guise of the Deceiver for item. DE pally "ftw," as they say.

I should note that Ghoulbane places a VERY close second. I used that sword for like 15 levels.
